What may cause a rash or severe Dhydration almost looking like a burn on the head of my Penis.?

It has an oily/scaly texture feel to it and it happens not very often i went to a dermatologist and they didnt rlly tell me nothin except use ointment which barely helped. Im honestly looking for a honest opinion Plz if you have any information answer back.
